Our thoughts are first and foremost with the affected patients and their families, who have endured distress and life-affecting consequences.

ANZSREI supports thorough, independent investigation into these incidents to identify the root causes and ensure that all necessary corrective actions are implemented. We encourage all fertility clinics to continue to review and reinforce their internal verification processes, patient confirmation safeguards, and electronic witnessing systems to prevent such devastating errors from recurring.
